History:
* Late 1960’s: The Austin neighborhood is the location of the organization’s two primary medical and counseling sites, predominately-African American. Almost half the population lives below 200 percent of the federal poverty level. Austin experienced major demographic changes, resulting in serious reductions in access to health and human services.

* 1974: Problems recognized and addressed by the congregation of Circle Church, resulting in the formation of Circle Urban Ministries, followed by the development of a health care delivery organization.

* 1975: Medical and behavioral healthcare professionals residing and attending churches in the Austin neighborhood convene to address local health care and counseling needs. Professionals moved to action by religious convictions and concerns over radical economic, racial and demographic changes over previous decade. Lack of behavioral health, counseling, family services and health care options within the community.

* 1977: Circle Family Care (CFC) becomes incorporated as Circle Family Care, with two independent agencies addressing the neighborhood's medical and counseling needs.

* 1984: The counseling and medical centers merged becoming a 501(c)(3) not-for-profit organization under the name Circle Family Care (CFC).

* 1986: Agency expanded its counseling services to the Humboldt Park community through a contract awarded by The Illinois Department of Mental Health. The expansion of services targets the African American and Hispanic populations residing in this neighborhood.
