History:
* Late 1960’s:
The Austin neighborhood is the location of the organization’s two primary medical and counseling sites, predominately-African American. Almost half the population lives below 200 percent of the federal poverty level. Austin experienced major demographic changes, resulting in serious reductions in access to health and human services.

* 1974: Problems recognized and addressed by the congregation of Circle Church, resulting in the formation of Circle Urban Ministries, followed by the development of a health care delivery organization.

* 1975: Medical and behavioral healthcare professionals residing and attending churches in the Austin neighborhood convene to address local health care and counseling needs. Professionals moved to action by religious convictions and concerns over radical economic, racial and demographic changes over previous decade. Lack of behavioral health, counseling, family services and health care options within the community.

* 1977: Circle Family Care (CFC) becomes incorporated as Circle Family Care, with two independent agencies addressing the neighborhood's medical and counseling needs.

* 1984: The counseling and medical centers merged becoming a 501(c)(3) not-for-profit organization under the name Circle Family Care (CFC).

* 1986: Agency expanded its counseling services to the Humboldt Park community through a contract awarded by The Illinois Department of Mental Health. The expansion of services targets the African American and Hispanic populations residing in this neighborhood. 
From a single site in 1977, our health services division has grown to four sites, housing a dedicated staff of highly trained specialists.
CFHCN Communities:
For over thirty years, CFHCN assumes the responsibility of providing the entire spectrum of clinical, social and spiritual support to the communities it serves. These include the community areas of Austin, Humboldt Park, East and West Garfield Park, and North Lawndale, with a predominately Latino population in Humboldt Park.
 
Our communities continue to rank among one of the top areas for mortality from all causes, including heart disease, stroke, lung cancer, diabetes mellitus and pneumonia, as well as homicide. These communities include:

    * Austin
    * Humboldt Park
    * East Garfield Park
    * West Garfield Park
    * North Lawndale


Chronic health care problems continue in our underserved communities, including diabetes, hypertension, asthma, cardiovascular diseases, sexually transmitted disease and escalating rates of HIV/AIDS, which has reached epidemic status. The majority of our patients are not insured. Their cases are quite complex, requiring extensive time, resources and medications. Seeing Hypertension and Diabetes are the top two reasons behind visits to Circle Family HealthCare Network. Our providers are compassionate dedicated about caring for patients with chronic disease as well as long-term health concerns.
